ADSR with delay and attack hold ("punch")

This is a module that sort of "fell out" of the development of my "Transient" module.
It is an ADSR envelope generator that also has controls for initial delay, attack hold (aka "punch") and sustain hold duration.
It includes all the other nice features that the Transient has: 1mS to 10S range on all controls, retrigger switch, LFO and ENV outputs, CV control of both output level and overall time.
