DIY CV Offset/Scaler module based on a quad OpAmp

Can offset and attenuate/amplify CV up to supply level. E.g. 0-5V from SQ-1 sequencer can 'play' the Phonogene's 'Slide' CV in which requires ~+/-8V for full range.
