This is the Westwood, EarthQuaker Devices' light- to medium-gain overdrive with active Bass and Treble controls, built by hand in Akron, Ohio. EQD no longer makes it.
Drive runs from a near-clean boost at low settings up to lead tones, and Level has plenty of output to push an amp. Bass works around 80 Hz and Treble around 2 kHz, each with up to 20 dB of boost or cut. It responds to your guitar's volume knob and your pick attack at any setting, and EQD says it works on bass too, with warm low mids and a smooth top end. Both jacks and the power jack sit on the knob end. It runs on a standard 9-volt center-negative supply, and the true bypass switching is by relay, so it needs power to pass sound.
